This template can be used to request donations for an Event from Dolphin Diver supporting Busunesses. Donations could include raffle prizes, food and venue use.
Please use this form to list details of your event for future event leaders. PLease include vendor contact information as well as any notes regarding your event that may assist future leaders. This form should be completed, printed and added to the Event binder for the event you are leading.
Please use this form to track your event income and expenses. After the event please submit a copy of this form with your event details and additional paper work (receipts, final contracts) to the secretray. Also keep a copy with the Event Binder for future event leader reference.
